CUTTING EDGE PET GROOMING — SERVICE AGREEMENT & LIABILITY WAIVER

1. GENERAL RELEASE & HEALTH DISCLOSURE: I certify that I am the legal owner of the pet being groomed and that all health, medical, and behavioral histories disclosed are accurate. I understand that grooming involves handling, sharp equipment, and bathing which carry inherent risks. I release Cutting Edge Pet Grooming from liability for any pre-existing medical conditions, stress-induced reactions, or injuries resulting from undisclosed health issues or old age.

2. MATTED COAT POLICY: I understand that severe matting traps moisture, restricts air flow, and can cause pre-existing skin irritation or infections. I grant permission to shave matted areas if necessary for my pet's comfort and health. I acknowledge that shaving a matted coat carries risks of minor nicks, razor irritation, or revealing underlying skin conditions, and I hold the salon harmless for such outcomes.

3. AGGRESSIVE PETS & SAFETY: I agree to disclose if my pet has a history of biting or aggression. The salon reserves the right to use humane muzzling or terminate services if a pet poses a safety hazard. I agree to pay all grooming charges up to the point of termination, plus any medical or damage expenses caused by my pet's aggressive behavior.

4. EMERGENCY VETERINARY CARE: In the event of a medical emergency, I authorize Cutting Edge Pet Grooming to transport my pet to an emergency veterinary clinic if I cannot be reached immediately. I agree to pay all expenses and fees incurred for necessary medical treatment.

5. FLEA & PARASITE POLICY: If fleas or ticks are discovered on my pet during the grooming process, a mandatory flea treatment bath will be administered at my expense.

6. PHOTOGRAPHY RELEASE: I grant permission for Cutting Edge Pet Grooming to take photos/videos of my pet during their visit for promotional, social media, or business record purposes.

BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ, UNDERSTOOD, AND AGREE TO ALL TERMS AND CONDITIONS STATED ABOVE.
